If you are a GP, school nurse or other health professional looking to refer a child, then you should use the Brighton & Hove Referral to Health Form, which can be faxed to us on 01273 431719.

If you have not used this form before, are not a health professional or wish to talk through a particular family’s situation, we encourage you to call us on 01273 431703.

The inclusion and exclusion criteria for children are based on a BMI for age centile assessment.

• Children with a BMI 91^st centile or over can be referred onto the Healthy Weight Referral Scheme and are then assessed by our trained co-ordinators.

• Children with a BMI 99.6^th centile or over need to be referred via their GP to a BSUH Paediatrician.
